There is a $100 charge if there are insurance forms to fill out. Let me explain how we can get started in getting you a custom pair. |
| In order for us to begin processing your order, and ship your casting kit
to you, we will charge your card $114 (which includes shipping) upon shipping of the casting kit. The casting kit includes: 1 casting foam box with instructions, Patient history form, shipping label. |
| Just follow the instructions on casting for the feet, then send the
entire casting kit back to us with the label supplied. Include the remaining payment or otherwise: |
| We will charge the remaining $295 CN when your custom made orthotics are shipped. |
| All orders are shipped via priority mail. Normal turn-around in our
lab is seven to ten days. Please allow two weeks from the time of your order for delivery of your orthotics. |
